Avalon
Psychiatry is located in the Park Plaza Office complex across the
street from the Olympic Village Shopping Center.
Directions to the office: From Highway 16, take
the Olympic Drive exit. If you are traveling East on 16 (towards
Tacoma) then you
will take a left at the light at the end of the exit ramp. If
you are traveling West on 16, take a right at the light at the
end
of the exit ramp. Go the the next traffic light past the overpass
(3rd light if approaching from East, 2nd light if traveling West
on Hwy 16) and make a right turn on to Hollycroft Street. Take
the first left off of Hollycroft into the Park Plaza parking
lot. Park Plaza East is the building closest to the street entrance.
There is a parking garage and lot parking available. From the
elevator
on the garage level, go to the 3rd floor, or you can go up the
outdoor stairs in the center of the complex (by the building
where John L. Scott Real Estate is also located) and walk up to
the next
floor. We are in Suite 310 East.

Office Hours: Appointments are available weekdays
with some late appointments and weekend appointments. Phone calls
are returned within
24 hours.
EMERGENCIES: If you are not
a current patient and have an emergency, call the crisis hotline
at 1-800-576-7764 or call 911 or go to the nearest hospital emergency
room. Current patients with an emergency that needs immediate response
may page Dr. Farrington at 1-877-294-4776. Otherwise please leave
a message on the voicemail and your call will be returned within
that working day.
Messages: Email is not encrypted/secured; email
and phone messages may be accessed by office staff.
Cancellations: 48-hour cancellation
policy. We reserve the right to charge for missed appointments.
Prescriptions: For refills,
you may call your pharmacy and have the request faxed to 253-851-9908
OR call the office and leave patient name, medication and milligram
dosage, pharmacy name and phone number, and your phone number.
Refills will be filled weekdays between 9 AM to 5 PM. Allow 48
hours for response.
Payment information: Payment
is due on date of service. Checks, debit and major credit cards
are accepted (MasterCard, Visa, Discover, American Express). If
you wish to submit billing to insurance, we will provide you with
necessary claim submission information. We do not bill your insurance
company.
Office Visits:
- Initial psychiatric evaluation (~60 - 90 minutes)
$250
- Medication management (~20 minutes) $110
- Medication with counseling (~30 minutes) $125
- Medical psychotherapy (~ 50 minutes) $155
- Group therapies (~75 minutes) $100 per patient
- Requests for letters, reports, records review and phone
consultation are charged according to time and complexity.
Privacy notification:
A record is kept of the health care services provided to you.
You have the right to see, copy and request corrections to the
records.
Your records can not be disclosed to others unless you direct
us to do so or unless the law authorizes or compels us to do
so.
